REQUIRED - Please read and click to agree: Floral deliveries will be received by the church office during normal office hours from 10 AM - 3 PM CT and placed in the area where the service will take place. Placement and removal of all florals are to be handled by the funeral home.

REQUIRED - Please read and click to agree: Minimal moving of Westside furniture and instruments is subject to approval. Any item(s) moved must be returned to their original positions.

Attachments

Recorded music or video slides to be played before or during the service must be submitted 48 hours before the funeral to Westside's Minister of Worship & Creative Arts for approval. The family can create a video slide show for the funeral, according to Westside's Media Ministry specifications, 72 hours before the funeral for review. If the family wishes Westside's media team to create the slide show, a cost will be determined (see Media Ministry guidelines).
